Job summary

The Department of Economic Security, Division of Developmental Disabilities, is seeking a Case Manager 2 to join its team in Prescott Valley. You will support individuals with developmental disabilities by developing Person-Centered Service Plans, coordinating services, and collaborating with families and community agencies.

Requirements include a Bachelor degree in Human Services or related field or RN, plus relevant case management experience; valid driver's license; background checks.

Qualifications

• Knowledge of HIPAA and health information privacy.
• Understanding of PCSP development and documentation.
• Familiar with federal/state statutes and agency policies.
• Strong case management principles.
• Ability to interview, counsel and engage with families.

Responsibilities

• Drive to members' homes and sites to discuss service needs.
• Develop, review, monitor PCSP and referrals.
• Facilitate meetings and program visits for member choices.
• Collaborate with community agencies for crisis support.
• Write progress notes and maintain case records.
