Job summary

The Department of Economic Security, Division of Developmental Disabilities, seeks a Case Manager 3 in Chandler to coordinate services for individuals with developmental disabilities. You will engage with families, arrange supports, and document progress to drive outcomes within a collaborative team.

This role emphasizes assessment, planning, and community collaboration, with on-site visits and regular reporting.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Special Education or Counseling.
• At least one year of case management experience for elderly or disabled individuals or individuals with serious mental illness (SMI).
• May require fingerprint clearance and background checks as part of pre-employment requirements.

Responsibilities

• Drive to members' homes for on-site visits and develop relationships to identify needs and supports.
• Attend meetings with members, providers, and others to coordinate services.
• Compile information to determine eligibility and re-determinations of eligibility.
• Write progress notes and maintain case records; prepare reports and participate in trainings.
