Job summary
Arizona Department of Economic Security (DES), Division of Developmental Disabilities, seeks a Case Manager 2 in Avondale, AZ to support individuals with developmental disabilities and their families. Responsibilities
- Drive to members' homes/sites to discuss services and supports.
- Develop, review, and monitor ISP within mandated timelines and referrals.
- Research community resources to meet ISP needs and inform members of options.
- Facilitate meetings and program visits to support member/family choices; complete referrals.
- Collaborate with community agencies to provide assistance during crises.
