Job summary

The Department of Economic Security (DES) is seeking a Case Manager 3 to join the Division of Developmental Disabilities (DDD) in Arizona. The role involves traveling to members’ homes, schools, and program sites to assess service needs and develop Person-Centered Service Plans (PCSP).

You will collaborate with community partners to support individuals with developmental disabilities and their families. The position requires licensure or a degree with relevant case management experience, a valid

Qualifications

Must be an Arizona licensed Registered Nurse or possess a Bachelor Degree in Social Work, Psychology, Special Education or Counseling, and have at least one year of case management experience for elderly or disabled persons or those with serious mental illness (SMI); or have at least two years of case management experience.
Driver’s License required for state business and driving is allowed, with proper insurance and driving record checks.
Employee must have a valid Level One fingerprint clearance card for work with children and vulnerable adults.
Background checks and registry searches (DCS Central Registry, Adult Protective Services) are required; employment contingent on successful completion of checks.
All new state employees must complete E-Verify as applicable.

Responsibilities

Drive a vehicle to conduct onsite visits to members’ homes, schools and program sites to discuss service needs and supports.
Develop, review, monitor and evaluate Person-Centered Service Plan (PCSP) with timely documentation and referrals.
Facilitate meetings and program visits to assist with member/family choices; complete referrals for assessed services.
Collaborate with community agencies to provide assistance and support to members and families in crisis.
Write progress notes and maintain case records; prepare reports and participate in meetings; may supervise in supervisor's absence.
