Job summary

The Department of Economic Security (DES) is seeking a Case Manager 3 for the Division of Developmental Disabilities in Chandler, AZ. You will coordinate supports and work with families to develop individualized plans that promote independence and community engagement.

Ideal candidates have a relevant degree or nursing license and at least one year of case management experience with seniors, disabilities, or mental health needs.

Qualifications

• Written and oral communication to convey a wide variety of information.
• Initiating, developing and maintaining member service plans.
• Interviewing and counseling with members and families.

Responsibilities

• Drive vehicle to conduct on-site visits to members/families’ homes and develop relationships.
• Convene and facilitate multi-disciplinary Individual Support Plan meetings.
• Provide crisis support and attend meetings with stakeholders.
• Prepare reports and maintain case records; participate in staffing and trainings.
