Job summary

Kindred Hospital Los Angeles is seeking a Care Coordination Specialist to oversee patient care through the Interdisciplinary Care Transitions team, ensuring efficient resource use and seamless transitions across the care continuum.

You will support discharge planning, collaborate with physicians, patients, families, and post-acute providers, and contribute to care optimization with excellent English communication and computer skills.

Qualifications

• Graduate of an accredited RN program.
• BSN preferred or MSW/BSW with licensure as required by state regulations.
• Knowledge of Medicare benefits and insurance processes.
• Experience with case management, utilization review, or discharge planning is preferred.
• Proficient in computer use and data recording (MS Office).

Responsibilities

• Coordinate clinical and psycho-social activities with the Interdisciplinary Team and Physicians.
• Monitor patient stays for effective care coordination and resource utilization.
• Refer high-risk patients to additional support when needed.
• Serve as a patient advocate and promote informed decisions.
• Participate in rounds to review goals and coordinate care across the continuum.
• Coordinate with payors, families, and post-acute providers for discharge planning.
