Damar Services is seeking a motivated Case Manager to lead, direct, and monitor treatment services for individuals with DD/ID. You will promote agency philosophy, handle all case management aspects, and collaborate with residential managers to implement IPPs.

Responsibilities include developing IPPs within 7 days of admission, coordinating with healthcare providers, and maintaining detailed case notes and data.

Qualifications

• Bachelor’s degree in education from an accredited program.
• Bachelor’s or Master’s in social work from an accredited program.
• Bachelor’s or Master’s in human services with 3+ years supervision or 1 year with DD/ID.
• Registered Nurse
• Therapeutic recreation specialist who is a graduate of an accredited program.
• Master’s degree in psychology from an accredited program.

Responsabilités

• Coordinate Independent Living goals and IPP development.
• Represent clients at appointments and exchange information accurately.
• Maintain service notes, contacts, data sheets and data collection.
• Plan and lead program meetings and IPP updates.
• Lead IPP implementation and staff training for assigned clients.
• Ensure monthly IPP updates by the 6th of every month.
• Review MHA, addenda, and complete ICAP assessments.
• Attend CFTMs, court reviews, and IEP/Case conferences as needed.
• Engage families and referral sources weekly for caseloads.
