Damar Services, LLC seeks a dedicated Case Manager to lead, direct and monitor treatment services for individuals with intellectual and developmental disabilities or autism in East Chicago, IN. The role requires collaborating with the care team, developing IPPs within 7 days of admission, and ensuring documentation and reporting for quality care.

Starting salary at $47,500+ depending on education and experience, with comprehensive benefits, tuition assistance, 401(k), and paid time off.

Qualifications

• At least one year of experience with DD/ID population or related training.
• Minimum age 21 for licensure requirements.
• Valid Indiana driver's license and driving record meeting insurance requirements.
• Clear background; pass pre-employment drug screen and physical.

Responsibilities

• Lead, direct, and monitor treatment services for assigned clients.
• Develop IPPs within 7 days of admission and update as needed.
• Maintain notes, data sheets, and data collection; prepare monthly IPP updates by the 6th.
• Collaborate with Residential Managers to execute IPPs and unit programming.
• Complete ICAP assessments within 30 days and annual Ansell Casey updates.
• Ensure family engagement and referral source communication weekly.
