Job summary

Damar Services is seeking a dedicated Case Manager to lead, direct, and monitor treatment services for individuals with developmental disabilities in Indiana. You’ll champion the agency philosophy and handle all case-management aspects for assigned clients, including IPP development and interdepartmental collaboration.

Qualifications

• At least one year of experience with DD/ID population or equivalent training.
• Minimum age 21 and valid Indiana driver’s license.
• Clear of felony convictions and passing pre-employment screenings.

Responsibilities

• Promote independent living skills and represent clients at appointments.
• Maintain service notes, data sheets, and data collection.
• Plan and lead Program meetings; develop IPPs within 7 days of admission.
• Collaborate with Residential Managers to ensure IPP execution.
• Lead IPP implementation and provide staff guidance and training.
• Update IPPs by the 6th of each month; review MHA and addenda as needed.
• Complete ICAP assessments within 30 days of admission; complete Ansell Casey assessments as needed.
• Monitor and report IL goals; ensure family engagement and referral communications.
• Attend CFTMs and represent clients in Court Reviews and IEP/Case Conferences.
