Job summary

Damar Services, LLC in Indianapolis is seeking a dedicated Case Manager to lead, direct, and monitor treatment services for individuals with intellectual and developmental disabilities. You will promote agency philosophy and oversee all case management activities for assigned clients.

Responsibilities include IPP development within 7 days of admission, collaboration with Residential Managers, and regular IPP updates, ICAP/Ansell Casey assessments, and family engagement.

Qualifications

• At least one year of experience with DD/ID population or equivalent training.
• Bachelor’s Degree in education from an accredited program.
• Bachelor’s Degree or Master’s Degree in social work from an accredited program.
• Bachelor’s or Master’s Degree in a human services field with 3+ years under supervision or 1 year with direct DD/ID work.
• Registered Nurse.
• Therapeutic recreation specialist with accredited program.
• Master’s degree in psychology from an accredited program.

Responsibilities

• Lead, direct and monitor treatment services for assigned clients.
• Promote agency philosophy in the delivery of services.
• Handle all case management aspects of assigned clients’ care.
• Represent clients at doctor’s appointments and exchange information accurately.
• Develop IPPs for each client within 7 days of admission and update as needed.
• Collaborate with Residential Managers to ensure unit programming.
• Write and update monthly IPPs to be ready by the 6th of each month.
• Complete ICAP assessment within 30 days of admission.
• Complete Ansell Casey assessment (within 20 days and annually).
• Monitor IL goals and monthly reports; ensure monthly IL reporting.
• Ensure family engagement communication and weekly referral source communication.
• Attend CFTM quarterly or as needed; represent clients in Court Reviews/IEP conferences.
