The position requires a Bachelor’s degree in social work or related field and 1 year of relevant experience. CPR/BLS/First Aid within 3 months and Spanish/English bilingual preferred.

Qualifikationen

• Bachelor’s degree in social work or related field is required.
• 1 year of work experience in a shelter, social services or nonprofit setting is required.
• CPR, BLS and First Aid certificates to be completed within 3 months of hire.
• Bilingual (Spanish/English) preferred.

Aufgaben

• Conduct comprehensive client assessments to identify needs, strengths, barriers, and eligibility for services.
• Develop, implement, and regularly update individualized service plans with clients.
• Coordinate access to community resources, benefits, and support services.
• Provide ongoing case monitoring and follow-up to ensure progress toward goals.
• Engage in frequent client contact via visits or telephonic support.
• Advocate for clients to ensure equitable access to services.
• Maintain confidentiality and accurate documentation per agency policies.
• Collaborate with staff and external partners to deliver coordinated care.
