Job summary

Interfaith Community Services in Escondido, CA is seeking a Case Manager 1/Senior Assistance to provide comprehensive case management for enrolled clients. You will assess needs, develop individualized plans, connect to community resources, and advocate for clients within program guidelines.

Role requires a Bachelor’s degree in social work or related field and at least 1 year in shelter or nonprofit settings; CPR/BLS/First Aid within 3 months; bilingual (Spanish/English) preferred.

Qualifications

• Bachelor’s degree in social work or related field, required.
• 1 year of work experience in shelter, social services or nonprofit settings, required.
• Valid CPR, BLS and First Aid certificates to be completed within 3 months of hire.

Responsibilities

• Conduct comprehensive client assessments to identify needs and eligibility.
• Develop and update personalized service plans with clients and teams.
• Coordinate access to community resources and support services.
• Provide ongoing case follow-up and monitor progress toward goals.
• Advocate for clients to ensure equitable access to services.
