Job summary

People Incorporated Mental Health Services in Minnesota seeks a Case Manager I for Targeted Case Management on a Metro County team. You will develop person-centered plans, connect clients to services, and work autonomously with a flexible schedule.

The role offers full-time hours, a hybrid location, and a hiring range of $20.75–$22.00 per hour plus a $1,000 external candidate bonus. Become part of Minnesota's largest nonprofit mental health provider.

Qualifications

• Bachelor's degree in behavioral sciences or related field or RN with psychiatry training.
• Experience in case management or related community mental health setting is preferred.
• Ability to develop person-centered support plans and coordinate services.

Responsibilities

• Perform functional assessments leading to individual plans and service access.
• Guide clients through applying for services and benefits.
• Attend bi-annual meetings to monitor services and outcomes.
• Build collaborative client relationships with positive, proactive communication.
• Assess needs and ensure continuity of care with outreach when needed.
• Maintain regular contact with partners and make referrals as needed.
• Make referrals and authorize services based on identified needs.
• Maintain records and document coordination activities accurately.
• Document interventions and data in the treatment plan per standards.
