Job summary

RADIAS Health is seeking a full-time Case Manager to join the Targeted Case Management team in a hybrid role. The position focuses on recovery and community integration for adults with serious mental illness and co-occurring substance use disorders in Ramsey County.

The role involves coordinating medical, social, vocational, educational, and housing services for clients. The starting salary range is $52,797 to $54,465 plus an office benefit package.

Qualifications

• Bachelor's degree in behavioral sciences or related field and 2,000 hours supervised work experience with individuals diagnosed with serious mental illness and/or substance use disorders
• Masters or other graduate degree in behavioral health sciences or related field
• If no degree, 4,000 hours supervised work experience with individuals diagnosed with serious mental illness and/or substance use disorders

Responsibilities

• Develop and implement the treatment plan based on assessment approved by clinical supervisor
• Provide or ensure access to medical, vocational, social, educational, financial, legal, housing, and chemical dependency services
• Develop an Individual Community Support Plan for each client every 6 months
• Schedule face-to-face visits as dictated by ICSP
• Make appropriate referrals and assist clients in gaining access to needed services
• Develop therapeutic relationships and complete documentation in an organized manner
