Case Manager II

Job Location: Peoples Incorporated Main Office – Eagan, MN 55121

Salary Range: $24.25 – $25.75 hourly

Hiring Bonus: $1,000 to external candidates

People Incorporated is seeking Case Manager II's to join our [Targeted, Waiver, Dual] Case Management [County] team to work with clients who are diagnosed with chronic mental illness. We support people in connecting with resources to live as independently as possible. We help them develop self-awareness to make positive plans for the future through frequent meetings in the community. Case Managers develop long term relationships with their clients and operate autonomously, creating their own schedules.

Main Job Duties

• Perform functional assessments that lead to individual plan development and result in the client receiving access to necessary mental health and related services.
• Conduct regular review of plan effectiveness and make adjustments as needed that result in the client receiving the maximum amount of assistance and support while monitoring the effectiveness of services received.
• Assess client needs and ensuring continuity of care.
• Make ongoing referrals to resources based on assessment and current client need.
• Maintain necessary daily records for continuity of service and reports to funding agencies.
• Excels at documenting all interventions, interactions and relevant information to the treatment plan consistent with professional and organizational standards.
• Attending client monthly meetings to monitor services.

Education and Experience Required for Case Manager II positions

• Bachelor’s Degree in Social Work, Special Education, Psychology, Nursing, Human Services or other fields related to the education or treatment of persons with developmental disabilities or related conditions, AND
• One year of experience in the education or treatment of persons with developmental disabilities or related conditions
