Job summary

GEO Reentry Services LLC is seeking a Case Manager to support participants in developing program plans and ensuring compliance within the facility. You will help residents build budgets, find housing, and engage in education and vocational programs to achieve financial independence.

Key duties include creating individualized plans, documenting progress, coordinating with the Job Developer, and maintaining confidential files while upholding policy requirements.

Qualifications

• Ability to write clear concise reports in English using correct grammar.
• Bilingual in English/Spanish preferred, but not mandatory.
• Ability to work with computers and the necessary software typically used by the department.

Responsibilities

• Prepare written personalized program for each resident with measurable goals.
• Assist residents to establish personal budget and housing, and enroll in education/vocational programs.
• Monitor facility sanitation and safety and assign daily chores to residents.
• Orient residents to facilitate smooth program entry.
• Maintain confidential resident files and ensure security of records.
• Verify jobs for residents and assist with employment opportunities.
• Prepare discharge reports upon resident release.
• Participate in staff meetings and contribute to problem solving.
• Maintain confidentiality of employee, resident and vendor information.
