Job summary

Colorado Coalition for the Homeless in Denver is hiring a Case Manager I who will collaborate with case managers, supervisors, health teams and partners to develop and implement care plans, coordinate services, and maintain documentation. The role emphasizes outreach, referrals, and crisis intervention to promote long-term housing stability for individuals and families.

You will deliver advocacy, assess needs, assist with benefits applications, and ensure data collection in the EHR.

Qualifications

• High School diploma or GED required; Bachelor's preferred.
• 1–2 years of experience with homeless population or related challenges.
• Basic Life Support Certification within 30 days.
• Colorado driver's license within 30 days of hire (with the exception of SSHC)

Responsibilities

• Manage a caseload of clients with high-service utilization and disabilities, including health needs.
• Coordinate care plans and referrals with community partners.
• Maintain client data and documentation in the EHR.
• Provide crisis intervention and safety planning.
• Advocate for clients and connect them with benefits like Social Security, Medicaid, and food assistance.
