Job summary

Colorado Coalition for the Homeless is seeking a Case Manager I to work with families, children, and individuals experiencing homelessness or at risk. You will develop care plans, coordinate services, monitor progress, advocate for clients, and maintain detailed case documentation across partnerships.

The role emphasizes motivational interviewing, housing support services, trauma-informed care, and strengths-based approaches to support long-term stability.

Qualifications

• High School Diploma or GED required.
• Bachelor’s degree in Psychology, Social Work, Human Services, or Criminal Justice preferred.
• Basic Life Support Certification within 30 days of employment.
• Must have Colorado driver’s license within 30 days of hire.

Responsibilities

• Manages a caseload of clients with high-service utilization and disabilities.
• Develops and implements care plans and coordinates services with partners.
• Maintains case documentation and data in the EHR system.
• Provides crisis intervention and advocacy to residents.
• Coordinates referrals to external services and community partners.
