Job summary

MidPen Housing Corp. in Palo Alto seeks a Case Manager I to engage supportive housing residents and provide services to help maintain stable housing and achieve self-sufficiency.

You will support residents toward independent living while coordinating with community resources and third-party partners. Responsibilities include delivering onsite programming, outreach to promote services, crisis support, documentation, and maintaining HIPAA compliance.

Qualifications

• MSW/MA or BA/BS in psychology, counseling, or related field with relevant experience in supportive housing.
• Experience serving high-need populations or homelessness is preferred.
• Ability to pass background and fingerprint checks; valid CA Driver’s License and transportation required.

Responsibilities

• Deliver onsite programming at assigned properties in alignment with internal standards and regulatory requirements.
• Conduct outreach via door-to-door, phone, email, and other methods to promote available services.
• Provide crisis support and follow mandated reporting procedures for incidents.
• Support residents with functional impairments through case management and independent living skills.
• Facilitate group and individual services to foster community engagement and residents' goals.
• Collaborate with third-party partners and local agencies to deliver and evaluate services.
• Maintain accurate records, ensure timely reporting, and uphold HIPAA standards.
• Travel between properties as needed.
