Job summary

Heading Home in Albuquerque seeks a Case Manager I to connect clients with housing, employment, healthcare, and other resources. You will conduct initial assessments, make referrals, and maintain confidential case records within our program.

You'll support clients in obtaining documents, managing money, and navigating services, including transportation needs. This full-time, non-exempt role requires strong organizational skills and the ability to work with diverse community partners.

Qualifications

• Excellent written and oral communication skills.
• Able to manage multiple tasks and deadlines.
• Experience engaging clients and coordinating services.
• Knowledge of community resources is preferred.

Responsibilities

• Determine clients' needs by conducting initial assessments.
• Provide referrals to housing, employment, healthcare and other services.
• Prepare and maintain confidential case records and DAP notes.
• Enter client data into electronic database/portal.
• Collaborate with service providers and community partners.
• Assist clients with obtaining documents (birth certificate, driver's license).
• Support clients with money management and independent living skills.
• Assist with housing opportunities and client status updates.
• Transport clients as needed to ensure program success.
