Job summary

Albuquerque Health Care for the Homeless is hiring a Community Support Worker to deliver trauma-informed, client-centered care to unhoused individuals across Albuquerque. You will manage a caseload of 20–30 clients, using company vehicles to reach clients and remove barriers to care and housing.

This is a full‑time, non‑exempt role within a multidisciplinary team. Candidate must have NM driver’s license (or be able to obtain within 30 days) and a relevant human services degree or equivalent

Qualifications

• Trauma-informed approach to client care.
• Harm reduction principles in service delivery.
• Ability to develop individualized service plans with client input.
• Clean NM driver’s license or ability to obtain within 30 days.

Responsibilities

• Assess individual client needs and goals.
• Create client-centered service plans with input from clients.
• Coordinate wrap-around services across AHCH programs.
• Refer clients to community resources and housing programs.
• Document client contacts accurately in EHR.
