Job summary

Acacia Network in Bronx, NY seeks a Sr. Housing Specialist to supervise social services in the shelter, ensure housing placements, and align care with DHS requirements.

You will oversee Intake and Case Management teams and report outcomes with the Program Director/Director of Social Service. The role requires at least five years' experience with homeless populations, expertise in substance abuse and mental health, and strong case documentation and collaboration with medical staff.

Qualifications

• Minimum of five (5) years’ experience working with homeless and/or mixed income population.
• Specialty in substance abuse and mental health issues
• Knowledge of crisis prevention, intervention, and resolution techniques

Responsibilities

• Mentor Case Managers and promote their professional development.
• Ensure Case managers maintain updated case files with necessary documentation.
• Conduct and document one-on-one supervision of direct reporting staff to assess and improve.
• Ensure clients have appropriate medical insurance and are referred to medical provider.
• Work with team to ensure building standards are met and conduct unit inspections and follow-up.
• Organize onsite and offsite training opportunities for professional development of staff.
• Participate and facilitate in interdisciplinary team meetings to discuss and overcome resident barriers.
