Job summary

Lao Family Community Development, Inc. (LFCD) is seeking a Case Manager in Oakland, CA to support low-income participants through WIOA and CalAIM initiatives.

The role requires coordinating with AJCC, training providers, and employers to advance job placement and retention outcomes. Responsibilities include comprehensive assessments, care planning, outreach, workshops, and maintaining accurate program documentation.

Qualifications

• Bachelor's degree in Social Work, Psychology, or related field; or 1 year case management experience.
• Bilingual Spanish/English desirable.
• Strong written and verbal communication.
• Ability to travel across the service area.

Responsibilities

• Conduct comprehensive participant assessments and develop care plans.
• Engage communities, recruit participants, and coordinate referrals.
• Deliver workshops, counseling sessions, and job readiness activities.
• Provide case management and employment retention services.
• Maintain participant records in CalJOBS, CMS, and databases.
• Build partnerships with employers, training providers, and agencies.
• Ensure compliance with HIPAA and funding requirements.
