Job summary

AbilityFirst in Long Beach, CA seeks a Community Navigator (Full-Time) to provide person-centered planning, case management, intake coordination, and community engagement for participants in licensed programs. This role requires collaboration with regional centers and families, and may require evening/weekend work.

The position emphasizes documentation, goal planning, and program integration, with a focus on outcomes aligned with participant goals.

Qualifications

• A High School diploma or equivalent is required.
• A Bachelor’s Degree in psychology, sociology, marriage & family, recreation therapy, etc or 12 ECE college units or 4+ years of related experience is preferred.
• Must be LIC Director Qualified in community center settings under Title 17/22 regulations is required.
• 2+ years of experience in developmental disability services or related field is required.
• Strong written and verbal communication skills are required.
• Valid Driver’s License is required.

Responsibilities

• Attend required 2-day Person-Centered Thinking training.
• Conduct screenings, phone calls and intake meetings as the first level of the intake process.
• Develop and implement Person-Centered Plans (PCPs) and 1-page descriptions.
• Ongoing goal development and daily goal documentation.
• Update annual forms/documentation (emergency contacts, consent forms, seizure plans, personal rights, etc.).
• Serve as a liaison to Regional Center Service Coordinators.
• Attend IPP meetings and submit required reports.
• Meet with participants monthly to review goals and placement.
• Coordinate community outings and volunteer activities.
• Input and maintain participant data in ProCare and ensure records are accurate.
