Job summary

The Bachrach Group in New York seeks a dedicated professional to conduct field outreach, perform bio-psycho-social assessments, and develop individualized service plans for individuals with substance use challenges.

You will provide intensive case management, coordinate with clinicians and social service providers, help clients access benefits and housing, and document progress in compliance with funder and agency requirements.

Qualifications

  • Bachelor’s degree in Social Work, Human Services, Psychology, or related field required; Master’s degree preferred.
  • Minimum 1–2 years of experience working with individuals with substance use disorders, co-occurring mental health needs, or homelessness.

Responsibilities

  • Conduct field-based outreach to identify and engage individuals with substance use challenges in community settings (streets, shelters, hospitals, and other service locations).
  • Complete comprehensive assessments, including bio-psycho-social evaluations, to determine client needs and develop individualized service plans.
  • Provide intensive case management, including linkage to detox, inpatient, outpatient, and harm reduction programs, as well as housing, medical, and social services.
  • Collaborate with interdisciplinary team members (clinicians, peers, social workers, and medical staff) to coordinate care and support client progress.
  • Assist clients in navigating healthcare systems, applying for benefits, and addressing barriers to recovery (transportation, identification, legal issues).
  • Monitor client progress through regular field visits, follow-ups, and documentation in accordance with program and regulatory requirements.
  • Advocate for clients to ensure access to necessary services while promoting harm reduction, recovery-oriented care, and relapse prevention.
  • Maintain accurate and timely case notes, reports, and client records in compliance with agency and funder standards.
