Qualifications

• Bachelor’s degree or relevant experience.
• 1–3 years of experience in Harm Reduction/Social services/community services.
• Excellent listening and written/verbal communication skills.
• Familiarity with trauma-informed care approach.
• Bilingual English/Spanish is a plus.

Responsibilities

• Conduct community outreach, provider education, and client recruitment for Harm Reduction Services.
• Provide case management to stabilize clients and address obstacles.
• Conduct assessments to determine clients’ needs for supportive services.
• Refer clients to Hyacinth staff and/or community agencies for psychosocial supports (housing, transportation, employment).
• Follow up with referrals to ensure needs are met.
