Zusammenfassung

Project Hospitality is seeking a Care Coordintor to manage a designated caseload within our Health Homes program on Staten Island. You will perform intensive care coordination, ensuring access to medical and social services per each client's care plan, and advocate for clients by phone and in person.

Under supervision of the Program Director, you will conduct home visits, participate in case conferences, and maintain timely documentation.

Qualifikationen

• Requires a Bachelor’s Degree or a High School Diploma with 2 years of care management experience.
• Experience working with homeless and HIV populations is required.
• Valid driver's license is required.

Aufgaben

• Provide intensive care coordination for a designated caseload.
• Ensure members access, engage, and retain services per care plan, including medical and social services.
• Advocate for clients by phone and in person; perform home visits and follow-up.
• Participate in case conferences and trainings; maintain chart documentation and billing logs.
