Description

Community Health Action of Staten Island (CHASI) drives dramatic improvements in the health of New Yorkers by feeding people who are hungry, healing families broken apart by violence, and bridging the gaps between people and the compassionate health care they deserve. CHASI serves the most vulnerable individuals, families, and communities with critical services and programs. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities – the poor and working poor, low income people with chronic illnesses, persons with criminal justice involvement, substance users, domestic violence survivors, people of color, and the LGBTQ community.

POSITION SUMMARY:

The Health Home program ensures persons living with chronic illness get the care and services they need. This may mean fewer trips to the emergency room or less time spent in the hospital, getting regular care and treatment from providers, or obtaining food and a safe place to live.

The Health Home Coordinator acts as program lead, supervises the Health Home team and manages daily program operations while ensuring the quality of care for clients and adherence to program standards. Supervision with the Care Management staff includes tracking of engagement, review of program documentation such as HARP eligibility Assessments or Plans of Care, and the provision of adequate clinical support to clinically complex clients. The Unit Supervisor is an integral part of the team in promoting program services, establishing referral sources and maintaining relationships with service providers to coordinate care for the community. The Health Home Coordinator will consistently exhibit core values of teamwork, advocacy and exceptional customer service and be an active member of CHASI’s Management Team.

DUTIES & RESPONSIBILITIES:

• Provide direct supervision to Care Managers for the development of effective interventions, key areas of performance and opportunities for skills building.


• Track client-level engagement and program performance.


• Ensure coverage of Care Manager’s caseload and provide case management activities as needed.


• Review and approve client documentation in line with program standards.


• Track HARP/CORE eligibility Assessments and POCs and other required documentation.


• Ensure that clients move through the HARP/CORE eligibility process to obtain HCBS services.


• Oversee the enrollment, eligibility, and case assignment process.


• Provide crisis intervention to clients in distress.


• Review and complete data reports to monitor program compliance, performance and key indicators.


• Drive quality assurance and quality improvement activities.


• Continuous review of the service delivery process to exceed customer expectations.


• Receive appropriate trainings in order to maintain clinical and programmatic standards.


• Facilitate staff meetings, supervision, Clinical Case review and training.


• Ensure staff are kept informed of policy and procedure and departmental priorities.


• Occasional community-based work is required to achieve client outcomes and health home standards.


• Perform all functions in alignment with CHASI’s Mission, Vision, and Core Values.


• Other duties as assigned.

Requirements

QUALIFICATIONS:

• At least three years of direct service experience in Health Home program or other program related to chronic disease management or other intensive case management.


• Bachelor’s degree required; significant experience that is additional to what is required above may be considered in lieu of a degree. Master’s degree may be considered in lieu of some years of experience listed above.


• At least 1 year of supervisory or management experience.

Position Status: Full Time

FLSA Status: Non-Exempt

Pay & Benefits: The pay range for this role is generally $52,000-$57,000 annually, commensurate with relevant experience and qualifications and in alignment with internal equity. Additionally, we offer an excellent benefits package that includes
