Job summary

Milwaukee Rehabilitation Hospital is seeking a Case Manager to provide individual program management for rehabilitation patients. You will collaborate with physicians to ensure patient progression through the continuum of care, managing clinical and financial treatment plans. Requires a current state licensure and a minimum of 3 years of healthcare experience, with preferred medical rehabilitation experience. Qualifications

• Current state licensure as a Registered Nurse, Licensed Social Worker, PT, OT, SLP.
• Minimum of 3 years healthcare and clinical experience in a hospital setting.
• 1 year medical rehabilitation experience preferred.
• Certification in case management preferred.

Job description

Position Summary:

The Case Manager, in collaboration with the physician,providesindividual program management for each patient toensure the patient's progression through the continuum of care in a manner that achieves the desired clinical andfinancial outcomes. Monitors and manages clinical and financial coordination of treatmentplanof assigned patientsto ensuretimely, cost-effective, individualized service delivery. Works with rehabilitation patients with variousdisabilities including, but not limitedto:spinal cord injury, brain injury, cerebrovascular accident, amputation,neurologic disorders, orthopedicconditions, and arthritis. Coordinates length of stay management within Medicare(CMS) guidelines and 60% compliance threshold.

Minimum Qualifications:

• Current state licensure as a Registered Nurse, Licensed Social Worker, PT, OT, SLP.
• Minimum of 3 years healthcare and clinical experience in a hospital setting.

Desired Qualifications:

• 1 yearmedical rehabilitation experience.
• Certification in case management preferred.

Knowledge,Skillsand Ability Requirements:

• Excellent communication, negotiation, and conflict resolution skillsrequired.
• Knowledge of reimbursement systems preferred.
