Qualifications

• Must hold current state licensure as RN or LSW; MSW without licensure considered.
• Minimum 3 years healthcare and clinical hospital experience.
• 1 year hospital setting experience preferred; case management certification preferred.

Responsibilities

• Provide individual program management for each patient to ensure progression through care continuum.
• Monitor and coordinate treatment plans to achieve clinical and financial outcomes.
• Coordinate CMS length-of-stay management and ensure timely, cost-effective service delivery.
• Collaborate with physicians and rehab team; coordinate community resources including vocational rehab.
• Ensure effective reimbursement processes and patient/ family communication.

Bradenton Rehabilitation Hospital is opening March 2026! Join our team to work in a beautiful new hospital with 60 private patient rooms and 2 rehab therapy gyms with cutting-edge technology.

The Case Manager, in collaboration with the physician, provides individual program management for each patient to ensure the patient’s progression through the continuum of care in a manner that achieves the desired clinical and financial outcomes. Monitors and manages clinical and financial coordination of treatment plan of assigned patients to ensure timely, cost-effective, individualized service delivery. Works with rehabilitation patients with various disabilities including, but not limited to: spinal cord injury, brain injury, cerebrovascular accident, amputation, neurologic disorders, orthopedic conditions, and arthritis. Coordinates length of stay management within Medicare (CMS) guidelines and 60% compliance threshold. Provides community resources as appropriate including vocational rehabilitation resources.

Pay: Rate of pay is based on years of experience and qualifications.

Minimum Qualifications

• Current state licensure as a Registered Nurse, Licensed Social Worker,
• MSW without licensure will be considered
• Minimum of 3 years healthcare and clinical experience in a hospital setting

Desired Qualifications

• 1 years experience in a hospital setting preferred
• Certification in case management preferred.
