Job summary

Clarksville Rehabilitation Hospital seeks a Case Manager to collaborate with physicians and manage individualized patient programs through the continuum of care, aiming for optimal clinical and financial outcomes.

You will monitor and coordinate clinical and financial aspects of treatment plans, ensure CMS-guided length of stay, and provide community resources to patients and families.

Qualifications

• Current state licensure as RN or LSW.
• 2 years hospital experience preferred.
• Case management certification preferred.

Job description

Position Summary

The Case Manager, in collaboration with the physician,providesindividual program management for each patient toensure the patient’s progression through the continuum of care in a manner that achieves the desired clinical andfinancial outcomes. Monitors and manages clinical and financial coordination of treatmentplanofassigned patientsto ensuretimely, cost-effective, individualized service delivery. Works with rehabilitation patients with variousdisabilities including, but not limitedto:spinal cord injury, brain injury, cerebrovascular accident, amputation,neurologic disorders, orthopedicconditions, and arthritis. Coordinates length of stay management within Medicare(CMS) guidelines and 60% compliance threshold. Provides community resources as appropriate including vocational rehabilitation resources.
Pay:Rate of pay is based on years of experience and qualifications.

Minimum Qualifications

• Current state licensure as a Registered Nurse or Licensed Social Worker.

Desired Qualifications

• 2 years experience in a hospital setting preferred.
• Certification in case management preferred.
