Job summary

Crossroads For Women Inc. in Windham, Maine is seeking a full-time Case Manager to support mothers and children within our residential treatment program. You will provide comprehensive case management, coordinate services, and advocate for access to resources in alignment with agency standards.

The role emphasizes trauma-informed care, collaboration with clinical and medical teams, and ongoing documentation in our EHR system to ensure quality, licensed, and CARF-compliant care.

Qualifications

• Bachelor's degree in social work or a related field; preferred but not required.
• Experience with children, child protective services or child welfare is a plus.
• Knowledge of crisis management techniques and behavioral health interventions.
• Strong communication and interpersonal skills; able to work independently or in a team.

Responsibilities

• Provide case management services to individuals and families in need.
• Complete documentation in the electronic health record (EHR) per standards.
• Conduct assessments to determine client needs and develop plans.
• Coordinate services with agencies and organizations; advocate for clients.
• Collaborate with clinical and medical teams to align care with policy.
