Job summary

Lifeworks is seeking a dedicated Case Manager in Westwood, MA to support adults with developmental and intellectual disabilities. You will coordinate services, write Day Habilitation Service Plans, and maintain progress notes with a focus on safety and compliance.

The role offers a Monday–Thursday 8am–4pm schedule, competitive salary of $50,473, and a strong benefits package including health insurance, retirement match, PTO, and tuition reimbursement.

Qualifications

• Experience working with individuals with ID/DD.
• Must be able to pass background checks.
• Excellent verbal and written communication skills.

Responsibilities

• Provide individual and small group instruction in self-care, activities of daily living, communication, social and motor skills.
• Provide direct care for ADLs and group activities; lift and transfer individuals as needed.
• Coordinate services with Interdisciplinary Team.
• Communicate respectfully with families, residential staff and participants’ teams.
• Develop and implement Day Habilitation Service Plans (DHSP).
• Maintain progress notes and document progress on objectives.
• Maintain participant records; file information current and complete.
