Lifeworks, Inc. is hiring a full-time Case Manager to work with adults with ID/DD in a mission-driven nonprofit. The role emphasizes advocacy, empowerment, and life-enhancement services in a supportive environment.

The position requires independent work ethic and a belief in improving lives, with a schedule of Monday–Thursday 8am–4pm and Friday 7:30am–3:30pm. Responsibilities include providing health-promoting supports, coordinating ISP plans, and maintaining confidential records while

Qualifications

• Experience working with Individuals with ID/DD.
• Bachelor's degree or an associate degree in a related field with two years of experience, or HS diploma and five years of related work experience in Human Services.
• Excellent verbal and written communication skills.

Responsibilities

• Promotes optimal health for individuals through routine, preventive, and emergency health care.
• Provides supervision, training and coaching to increase self-sufficiency and independence.
• Develop, write and present annual ISP plans in consultation with supervisor.
• Maintains confidential files and submits required paperwork according to agency procedures.
