• Experience working with Individuals with ID/DD.
• Excellent verbal and written communication skills.

Responsibilities

• Promotes health and safety in the work environment.
• Provides supervision, training and coaching across Life Enhancement Program activities.
• Develops, writes and presents annual Individual Support Plans (ISP).
• Performs case management and maintains confidential files.
• Maintains communication with individuals, families, and teams to ensure consistency.
