Five Horizons Health Services (FHHS) in Tuscaloosa, AL seeks a Case Manager to coordinate HIV-related social services as part of an interdisciplinary health team. You will activate client paperwork, arrange transportation and financial aid, and support enrollment in ADAP/AIAP while ensuring timely, accurate documentation.

The role requires a Bachelor's degree in a social science or health-related field (or equivalent) and strong communication, organization, and detail-oriented skills to navigate

Qualifications

• Bachelor's degree in a social science or health-related field or equivalent experience.
• One year of direct HIV case management experience preferred.
• Excellent interpersonal and written/verbal communication skills.
• Ability to work in a fast-paced environment and manage multiple tasks.
• Detail-oriented and well organized.

Responsabilités

• Complete initial paperwork for activating clients and maintain accurate records.
• Coordinate transportation, financial and medication assistance, and ADAP/AIAP enrollment.
• Provide housing assistance and resource information.
• Maintain positive communication with clients and work as part of the treatment team.
• Assist the Director with state reports and interns.
