## Description**Position Purposes**: The Case Manager is a motivated and compassionate individual responsible for the daily operation and execution of the social services as well as coordinating with other departments to ensure clients are successfully engaged. The Case Manager works as part of an interdisciplinary health team to coordinate linkage to care, identify and minimize barriers to care, and facilitate the ongoing delivery of social services to clients of Five Horizons Health Services (FHHS).**Essential Functions:*** Ensures that initial paperwork for activating clients is completed, maintained, and filed in an accurate and up-to-date manner.* Participates in HOPE Clinic and other structured activities with Whatley Health Services, INC.* Coordinates/Arranges transportation, financial assistance, medication assistance, food assistance, and ADAP/AIAP enrollment.* Provides housing assistance and/or resource information.* Maintains positive, supportive, and productive communication with clients.* Works cooperatively as a member of the Treatment Team to address clinical and policy issues.* Actively participates in preparation and facilitation of site visits by funding agencies.* Maintains relevant training and licensures.* Assists the Director of Social Services with state reports.* Assists the Director of Social Services with interns.## Qualifications**Education/Professional:*** Bachelor's degree in social science or health-related field with previous experience in HIV care *or** Bachelor's degree (any field) with one (1) year of direct HIV case management experience* Excellent interpersonal skills* Ability to work in a fast-paced environment* Excellent written and verbal communication skills**Physical Requirements:** This position requires an individual who is self-motivated and able to independently complete assigned responsibilities.
