DAP Health is seeking a Medical Case Manager to coordinate medical and social services for individuals with HIV, ensuring access to care and continuity through a coordinated care plan. The role involves assessing clients, documenting in the ISP, and liaising with community services to advocate on behalf of clients.

The position requires a Bachelor's degree in a related field, 2+ years of HIV service experience, and a current BLS certification. Bilingual Spanish is preferred.

Qualificações

• Bachelor's degree in social work, psychology, healthcare or related fields.
• Minimum of two years’ experience delivering services to people living with HIV/AIDS in ASO or related social services organization.
• Current BLS certification through American Heart Association or American Red Cross.

Responsabilidades

• Coordinate delivery of medical and social services for persons with HIV infection.
• Assess client mental, social, financial, and functional status and document in ISP.
• Act as liaison between client and community social services; advocate as needed.
