Job summary

Indiana Professional Management Group, Inc (IPMG) is seeking a Case Manager for a full-time hybrid role based in Indiana. You will travel to community meetings multiple times per week and work remotely from a home office.

Applicants must reside in Indiana or within 10 miles of the state line. IPMG offers an ESOP, a $500 sign-on bonus within the first 6 months, robust training, PTO, holidays, and mileage reimbursement.

Qualifications

• Bachelor's Degree in Human Services is required.
• Minimum of one year of professional experience with Individuals with intellectual and developmental disabilities.
• Valid driver’s license, car insurance, and reliable transportation.

• Create individual support plans based on individuals' needs and goals.

What You’ll Do

• Provide information and education to help Individuals understand the Medicaid waiver process
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
• Help Individuals access services and supports they need to meet their goals
• Facilitate regular community‑based team meetings to review plan of care and collaborate with service providers as needed

Who You’ll Work With

• Case managers work with Individuals on two Medicaid Waivers; Family Supports Waiver (FSW) and Community Integrated Habilitation Waiver (CIH)

What You Need

• A valid driver’s license, car insurance, and reliable transportation
• A Bachelor’s Degree in Human Services and a minimum of one year of professional experience working with Individuals with intellectual and developmental disabilities
• Designated home office setting with computer, internet service, and smart phone
