Job summary

IPMG is seeking a Case Manager for a full-time, hybrid role in Indiana. The position requires travel to community meetings several times per week and working remotely from a home office. In-person meetings are required, and applicants must live within Indiana or within 10 miles of the state line.

The role focuses on educating Individuals about the Medicaid waiver process, developing tailored support plans, and coordinating access to needed services with a team of providers.

Qualifications

• Bachelor's degree in a human services field is required or equivalent.
• Valid driver's license, car insurance, and reliable transportation.
• Ability to pass thorough background screens.

What You’ll Do:

• Provide information and education to help Individuals understand the Medicaid waiver process
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
• Help Individuals access services and supports they need to meet their goals
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed

Who You’ll Work With:

• Case managers work with Individuals on four Medicaid Waivers; Family Supports Waiver (FSW), Community Integrated Habilitation Waiver (CIH), Health & Wellness Waiver (H&W), and Traumatic Brain Injury Waiver (TBI)

What You Need:

A Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field, or

A Bachelor's degree in an unrelated fieldanda minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population.
