Job summary

IPMG is seeking a Case Manager in Indiana for a full-time, hybrid role. You will travel to community-based meetings several times per week and work remotely from a home office.

Applicants must reside in Indiana or within 10 miles of the state line and participate in in-person meetings as required. The position offers a strong training program, growth opportunities, paid time off and holidays, and reimbursement for expenses and mileage.

Qualifications

• Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field.
• Bachelor's degree in an unrelated field and a minimum of one year of direct experience with people with intellectual/developmental disabilities, or two years with aging population.
• Valid driver's license, car insurance, and reliable transportation.
• Ability to pass thorough background screens.
• Designated home office with computer, internet and a smartphone.

Responsibilities

• Provide information and education to help individuals understand the Medicaid waiver process.
• Create an Individual Support Plan based on the strengths, needs, goals, and desires of the individuals we serve.
• Help individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.

• Case managers work with Individuals on four Medicaid Waivers; Family Supports Waiver (FSW), Community Integrated Habilitation Waiver (CIH), Health & Wellness Waiver (H&W), and Traumatic Brain Injury Waiver (TBI)

What You Need:

A Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field, or

A Bachelor's degree in an unrelated fieldanda minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population.
