Job summary

Indiana Professional Management Group Fort Wayne, IN is seeking a full-time Case Manager to join our hybrid team. The role requires travel to community-based meetings several times per week and working remotely from a home office.

Applicants must live in Indiana or within 10 miles of the state line and hold a Bachelor's degree in a human services field or related experience. You will educate families about Medicaid waivers and develop individualized support plans.

Qualifications

• A Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field.
• Or a Bachelor's degree in an unrelated field with at least one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population.
• Valid driver's license, car insurance, and reliable transportation.
• Ability to pass thorough background screens.
• Designated home office setting with computer, internet service, and smartphone.

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process.
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve.
• Help Individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.

Who You’ll Work With:

• Case managers work with Individuals on four Medicaid Waivers; Family Supports Waiver (FSW), Community Integrated Habilitation Waiver (CIH), Health & Wellness Waiver (H&W), and Traumatic Brain Injury Waiver (TBI)

What You Need:

A Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field, or

A Bachelor's degree in an unrelated field and a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population.
