Job summary

Indiana Professional Management Group is hiring a Case Manager in Kentland, IN. This full-time, hybrid role requires travel to community-based meetings several times weekly and remote work from a home office. Applicants must live within Indiana or within 10 miles of the state line.

Qualifications

• Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field.
• Or a Bachelor's degree in an unrelated field with a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years of experience with aging population.
• A valid driver's license, car insurance, and reliable transportation.
• Ability to pass thorough background screens.

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process.
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve.
• Help Individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.

Who You'll Work With:

• Case managers work with Individuals on four Medicaid Waivers; Family Supports Waiver (FSW), Community Integrated Habilitation Waiver (CIH), Health & Wellness Waiver (H&W), and Traumatic Brain Injury Waiver (TBI)

What You'll Need:

A Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field, or

A Bachelor's degree in an unrelated fieldanda minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population.
