Job summary

PAM Health Rehabilitation Hospital of Tavares is seeking a Case Manager to coordinate inpatient care across disciplines and ensure appropriate utilization of hospital resources. The role includes assessing discharge needs and coordinating discharge planning while collaborating with an interdisciplinary team to achieve optimal outcomes.

The Case Manager will maintain and enhance payor relationships, requiring state licensure and current BLS certification. Prior inpatient experience is preferred.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Professional disciplines may include OT, PT, RN, RRT, SLP, or LSW.
• Current BLS certification required.

Responsibilities

• Coordinate care across disciplines for designated patient populations.
• Review inpatient admission records to ensure appropriate level of care and utilization.
• Assist with discharge planning and coordinate post-discharge needs; maintain payor relationships.
