Job summary

The Salvation Army in Daytona Beach, FL seeks a Full Time Regular Case Management professional to conduct client interviews, develop intake and case plans, maintain HMIS records, and coordinate housing and community referrals. The role emphasizes client progress, documentation, and collaboration with community agencies.

Ideal candidates will have two years of college coursework in social work or related fields and two years’ experience in social/public service settings.

Qualifications

• Two years of college coursework from an accredited college or university in Social Work, Behavioral Science, or a related field.
• Two years’ experience working in a social or public service environment with experience assisting the public or an equivalent combination of education and experience.

Responsibilities

• Interviews applicants to determine eligibility for program enrollment based on established guidelines.
• Develops intake and case plans for clients and completes necessary paperwork.
• Prepares and maintains case records and HMIS entries.
• Meets regularly with clients to discuss progress and document outcomes.
• Plans and facilitates life management classes to develop client skills.
• Assists clients in locating affordable housing and coordinates housing resources.
• Refers clients to appropriate programs or other agencies as needed.
• Completes discharge plans and maintains follow-up records.
• Attends community meetings to educate on the program and promote referrals.
• Prepares and maintains monthly statistics reports.
