Job summary

Qualifications

• Active CDCA certification or ability to obtain within 30 days of hire.
• High school diploma or equivalent.
• Valid driver's license.

Responsibilities

• Maintain assigned caseloads and ensure regular client contact.
• Monitor client progress and assess ongoing needs per policies and contracts.
• Meet with clients individually and in group settings.
• Collaborate with families, clinicians, case managers, and community partners.
• Provide crisis intervention, advocacy, resource coordination, and assistance with ADLs.
• Develop service plans and complete clinical documentation using EHR systems.
• Maintain accurate, timely, and compliant client records.
• Communicate progress with probation departments and referral sources as appropriate.
• Participate in required trainings, staff meetings, and agency events.
