Resources for Human Development (RHD) FaSST/Connections CTI in Philadelphia, PA, is hiring a Resource Coordinator / Case Manager for a full-time role at $23.85/hour with DailyPay and comprehensive benefits. You will support individuals experiencing homelessness through service coordination and advocacy for stability.

You will assess needs, link housing, mental health, and community resources, develop strength-based goals, document progress, and collaborate with shelters and providers to ensure

Qualifications

• Bachelor's Degree in a Human Services field is required.
• Strong verbal and written communication skills are essential.
• Ability to work independently and with a multidisciplinary team.
• Experience with trauma-informed, strength-based support is preferred.
• Proficiency with computers, EHRs, and remote communication tools.

Responsabilités

• Assess participant needs and connect individuals to appropriate services and resources.
• Coordinate mental health, housing, education, vocational, medical, and community services.
• Develop and maintain strength-based personal goal plans.
• Monitor service delivery and participant progress.
• Collaborate with shelters, treatment providers, and community organizations.
• Assist participants in accessing benefits and entitlements.
