Job summary

Humana is seeking a dedicated Case Manager to support Wisconsin Family Care members in Dane/Columbia counties. You will assess needs, develop a comprehensive care plan, and work with a Field Care Nurse to deliver services in the least restrictive environment, focusing on high-quality, person-centered support.

You will perform enrollment assessments, regular home visits, and ongoing outreach. Travel up to 40% and strong documentation skills are essential to success in this remote role.

Qualifications

• Bachelor's degree in human services or related field with at least 1 year of experience serving frail elders or adults with disabilities, or a Bachelor's in another field with at least 3 years of relevant experience.
• Valid state driver's license and proof of personal vehicle liability insurance with 100/300/100 limits.

Responsibilities

• Assess members health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to address health and safety needs in the least restrictive environment.
• Conduct face-to-face social assessments at enrollment and at least every six months, typically at the member's residence.
• Conduct quarterly in-person visits and maintain monthly contact by phone.
• Arrange non-medical support services, including social integration, community resources, housing, and employment.
• Maintain accurate documentation including MCP updates and service authorizations.
