Job summary

Humana is seeking a Case Manager for the Wisconsin Family Care (FC) program to deliver comprehensive case management to frail elders and adults with IDD or physical disabilities in Dane/Columbia counties and surrounding areas.

You will assess health and safety needs, develop a Member Care Plan, and coordinate with a Field Care Nurse to address services in the least restrictive environment. Travel up to 40% is required.

Qualifications

• Bachelor's degree in human services or related field required with 1 year of relevant experience; or 3 years with a BA in another field.
• Valid state driver's license and proof of personal vehicle liability insurance (100/300/100).

Responsibilities

• Assess health and safety needs to develop a comprehensive Member Care Plan.
• Coordinate with a Field Care Nurse to deliver services in the MCP.
• Conduct face-to-face social assessments at enrollment and at least every 6 months.
• Perform quarterly in-person visits and monthly phone contact with members.
• Arrange non-medical services including social integration and housing.
• Maintain documentation like case notes and MCP updates.
