Qualifications

• Bachelor's degree in human services or a related field required.
• 1 year of experience serving frail elders or adults with IDD/physical disabilities, or 3+ years in another field.
• Valid state driver's license and proof of auto liability insurance (100/300/100).

Responsibilities

• Assess health and safety needs to develop a comprehensive Member Care Plan.
• Coordinate with a Field Care Nurse to deliver services in least restrictive environment.
• Conduct face-to-face social assessments at enrollment and at least every six months.
• Perform quarterly in-person visits and monthly phone contact with members.
• Arrange non-medical supports including social integration, housing, and community resources.
• Maintain accurate documentation: MCP, service authorizations, and case notes.
